Immunizations & Physical Exams / Vacuna & Exámenes Físicos

At Governor Pritzker's press conference announcing that Illinois will enter Phase 4 on Friday, June 26, Dr. Ezike encouraged Illinois residents to make appointments to visit their doctors now. We would like to remind parents/guardians that Illinois School Code requires all students entering preschool, kindergarten and 6th grade to receive physical exams and the required immunizations before the school year begins. This is also true for all students who are new to ASD4. Children who do not have their immunizations and physicals on file prior to the first day of school will be excluded from school until these records are up to date.
In addition, an athletic permit card is required of all students participating in Indian Trail Junior High School athletic programs.

Food/Produce pickup Friday, June 12

Produce and milk will be available for pick-up at the Addison Township Food Pantry on Friday, June 12 from 10:00-11:30 a.m. on a first-come, first-served basis. The Addison Township Food Pantry is located at 50 E. Oak Street in Addison.

Addison School District 4 will offer a prorated refund to any students that paid for transportation for the 2019-2020 school year. Prorated refund as follows:

·       Families that paid $100 transportation fee will receive a refund of $27.00

·       Families that paid $500 transportation fee will receive a refund of $135.00
